S.U.C.C.E.S.S. Multi-Level Care Society manages the Simon K.Y. Lee Seniors Care Home, which is named in honour of its key supporter, Mr. Simon K.Y. Lee. This long-term care facility offers culturally-appropriate professional care and supervision to adults in a supportive and secure environment.
Services are available to people with complex care needs, and physical and/or mental health conditions who cannot live safely and independently at home. The Care Home was opened on September 24, 2001.
The Province of British Columbia contributed $14.52 million for the construction of the Care Home, and Vancouver Coastal Health funds its daily operation. S.U.C.C.E.S.S. Multi-Level Care Society raised $3 million to augment government funding for the construction cost of the Care Home.
The Care Home is open to any individual eligible for complex care services under The Ministry of Health.
Please contact the local Home Health and Community Care Office in your area and speak to the intake/liaison nurse at the unit. The nurse would assess if the client is eligible for admission to a care home.
