S.U.C.C.E.S.S. Simon K.Y. Lee Seniors Care Home

S.U.C.C.E.S.S. Multi-Level Care Society manages the Simon K.Y. Lee Seniors Care Home, which is named in honour of its key supporter, Mr. Simon K.Y. Lee. This long-term care facility offers culturally-appropriate professional care and supervision to adults in a supportive and secure environment.

Services are available to people with complex care needs, and physical and/or mental health conditions who cannot live safely and independently at home. The Care Home was opened on September 24, 2001.

The Province of British Columbia contributed $14.52 million for the construction of the Care Home, and Vancouver Coastal Health funds its daily operation. S.U.C.C.E.S.S. Multi-Level Care Society raised $3 million to augment government funding for the construction cost of the Care Home.

Services

  • Professionally directed care 24 hours a day, 7 days a week for residents who have complex care needs
  • Special Care Unit for cognitively impaired residents requiring special environmental adaptations; others are for complex care residents
  • Full nutritional and food services as well as recreation programs
  • Medical services provided by individual family physician
  • Housekeeping and routine laundry services
  • Basic cleaning and maintenance of wheelchairs
  • Other specialized service e.g. palliative care as needed by the resident
  • Four self-paid beds available for eligible clients who require long or short term care in a safe and therapeutic environment
  • Bi-lingual (English and Chinese) staff

Facilities

  • The Care Home, measuring 70,000 sq. ft., is a three-storey concrete building with five resident units – Pine Garden, Bamboo Garden, Cherry Garden, Birch Garden and Cedar Garden.
  • There are a total of 114 beds in private ensuites (104 single and 5 double rooms).
  • Rooms are fully furnished with basic furniture and measures about 215 sq. ft. to 356 sq. ft. for double occupancy.
  • Each resident unit is equipped with a spacious living room, dining room, activity room, bathroom and kitchenette.
  • A hair dressing salon and a dental unit are situated on the ground floor.
  • Beautiful patios and landscaped garden for residents to enjoy gardening and outdoor activities.

Fees for Funded Beds

  • Residents pay 80% of their after tax income, which covers the cost of food and accommodation.
  • Minimum and maximum rates apply.
  • There may be other costs not included in the resident contribution.

Fees for Self-Paid Beds

  • $9,200 per month (subject to change with notice).
  • Extra charges may apply depending on the care needs of the resident.

Eligibility

The Care Home is open to any individual eligible for complex care services under The Ministry of Health.

Please contact the local Home Health and Community Care Office in your area and speak to the intake/liaison nurse at the unit. The nurse would assess if the client is eligible for admission to a care home.

Registration

The following registration procedures apply as required by Vancouver Coastal Health:

  • Contact your local Home Health and Community Care Office.
  • A case manager will determine your needs and eligibility.
  • Consent is required for admissions to long-term care homes.
  • When offered a bed in client’s preferred care home, the client will be expected to move with very short notice – often within 48 hours of a bed becoming available.

While COVID-19 can make anyone sick, our seniors with specific health circumstances are at an increased risk of more severe outcomes. To protect our seniors and staff, we have implemented strict infection control practices, restricted visitation by families and our staff are restricted to work at one site only to prevent community spread.
